A. PREPARATORY
- The child must be 4 years of age.
- Must have passed the interview.
- Must submit a copy of National Statistics Office (NSO) Birth Certificate.
B. KINDERGARTEN
- Must have attended Preparatory.
- The child must be 5 years of age.
- Must have passed the interview.
- Must submit a copy of NSO Birth Certificate.
C. ELEMENTARY (Grade 1)
- Must have attended Kindergarten.
- Must not below six (6) years old at the time of Enrolment.
- Must submit a report card duly signed by the Principal, NSO Birth Certificate (Photocopy) and a CERTIFICATE of Moving-Up as a Kindergarten Graduate.
C. HIGH SCHOOL (Grade 7)
- Must have passed the Forbes Academy Entrance Test.
- Must have passed the interview by the Principal/ Guidance Coordinator/ Staff.
- Must submit the following:
- Report Card duly signed by the Principal/ Director
- Certificate of Moral Character
- Photocopy (NSO Birth Certificate)
- 2 copies of recent 2X2 photo
C. HIGH SCHOOL (Grade 11)
- Must have passed the Forbes Academy Entrance Test.
- Must have passed the interview by the Principal/ SHS Coordinator/ Guidance Coordinator/ Staff.
- Must submit the following:
- Report Card duly signed by the Principal/ Director
- Certificate of Moral Character
- Photocopy (NSO Birth Certificate)
- Medical Certificate
- 2 copies of recent 2X2 pictures
F. STUDENTS FROM OTHER SCHOOLS TRANSFERRING TO FORBES
- Must have an Average Grade of not lower than 82 in all subjects.
- Repeaters are not accepted.
- Must have passed the qualifying examination and interview of the Guidance coordinator or Principal.
- Must have a grade of 85 or above in Attitude/ Conduct.
- If qualified, they must submit the following:
- Certificate of Moral Character coming from the Principal and Guidance Counselor of the school last attended.
- Form 138 and/ or Form 137
- NSO Birth Certificate (Photocopy)
- 2 copies of a 2×2 photo
- Recommendation from the Guidance Counselor/ Principal.
